What is a Physical Therapist?

Physical Therapists are university trained and are experts in injury diagnosis, injury treatment, exercise prescription, injury prevention, rehabilitation and many other areas of sport and health and fitness.

Do I need a prescription for physical therapy?

Yes, other than the HMO policies that require authorizations. Our clinic requires you to have a prescription to treat.

You have multiple offices. Can I go between offices??

Yes, we are one business and your medical records and schedule can be accessed at any of our locations for your convenience.

What can I expect as a new patient?

Our front desk staff will ask you to fill out new patient and health history forms. We ask that new patients arrive 30 mins before scheduled appointment time to complete your paperwork. These forms may be accessed on our website under Patient Info/New Patient.

Does my insurance cover massage therapy?

Most insurances cover MANUAL THERAPY but it would have be deemed medically necessary by your therapist.

What should I wear to my appointment?

Comfortable clothes. If you forget, no worries we have some loaner clothes you can borrow.

We collect patient feedback on an ongoing basis to help improve our services and ensure our patients receive the best care possible. We offer our patients several options for giving feedback.

Please write a list down of all medications you are currently taking and bring that with you. Also bring with you your ID and insurance card or copy of your card. If your insurance requires a co-pay/deductible/co-insurance, please be prepared to pay that at the time of your visit. In addition, it is helpful to write down a list of questions for your provider so that we can ensure that we answer all of your questions in the exam room.

Obtaining a copy of your medical record is easy. To start your request, simply download, print, complete, and sign the Authorization for Disclosure of Protected Health Information Form. Fax or mail them back to us, to the attention of the Medical Records Department. It will take 15 business days after payment is received.

Fees:
Patient access fees - is charged $.25 per page, plus postage (if mailed), and administration fee of $25
Physician access fees - there is no charge if requested by a provider with authorization.

Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan. We are only able to provide quotes for your visit. If the services are not covered, we can work with you to develop a payment plan.

Forms can be dropped off at the front desk. Depending on the type of form, like Disability and Life Insurance may take up to 7 business days to be completed after payment is received of $25. Some paperwork might require an appointment with your therapist for completion.
